The Process:
To achieve the best results, it is necessary for you
to take a large role in your goal setting,
performance, and feedback. Below you can read an
outline of what to expect from your initial contact
to the day of discharge.
Step 1
Verification

Phone or walk in with your referral/prescription
to schedule an initial evaluation

Submit insurance information and we will verify
your benefits. It is our policy that you know
your financial responsibility BEFORE you are
treated.

Fill out paperwork –the packet can be obtained
in person, or printed from our website.

Thorough Initial
Evaluation

Functional deficits are identified and therapy
goals are set by you and a licensed therapist

When possible, your therapist will attempt to
instruct and issue you a written home exercise
program

Patient representative contacts you within 24-48
hours to introduce themselves and address
questions or concerns

3 Follow-up
Visits
-
Treatment program is progressed with the intent
to make you independent with your home program

Close supervision of treatment by the therapists
to ensure proper form and maximum results

Constant open line of communication to progress
your treatment and educate you about your
condition. Therapists constantly asking you
for feedback about what you are feeling during
treatment

Discharge

Goals have been achieved, or partially achieved,
with the understanding that continuation of the
home program will deliver the desired results

Patient has been progressed to a thorough and
appropriate home program which is tailored to
maintaining results and improving
performance/quality of life
After your treatment is complete, you are welcome to
contact us at any time and for any reason,
including: a copy of your home treatment program,
questions or concerns, referrals, general health
information, etc. If we do not have the answer
immediately, we will work diligently to find you a
solution in the shortest amount of time.
